I hereby authorize and irrevocably grant to the Children’s Defense Fund, and Welcome Table, Inc., and their affiliates, licensees, agents and assigns the unrestricted right to use and publish any part of the information I have given to the Children’s Defense Fund and the right to record my name, voice, appearance, likeness, and comments on film, videotape, audiotape, still photographs, print and any other media now known or hereafter invented. I acknowledge that the Children’s Defense Fund and Welcome Table, Inc. shall own all right, title and interest in and to this media. I further agree that the Children’s Defense Fund and Welcome Table, Inc. may cause all or parts of this media to be used for any and all publications, exhibitions, public displays, editorials, advertising or other purposes.
I waive any inspection or approval of the media or any advertising or publicity in which my name, voice, appearance, likeness, narrative or comments might appear. I expressly release and agree to hold harmless the Children’s Defense Fund and Welcome Table, Inc., and their agents, employees, licensees and assigns from and against any and all claims including, but not limited to, invasion of privacy that I might ever have in any way relating to my image, recording, likeness, or interview, or their use.
Por la presente autorizo e irrevocablemente doy al Fondo Para la Defensa de Niños, sus afiliados, titulares, agentes y asignados el derecho sin restricción a usar y publicar cualquier parte de la información que yo he dado a CDF. Igual doy el derecho a grabar el nombre de mi hijo/a, su voz, apariencia, semejanza y comentarios en película, grabaciones de video y cinta de audio, fotografía, imprenta u otros medios aun no conocidos o inventados en un futuro. Yo reconozco que CDF poseerá todos los derechos, titulo e interés en y a estos medios. Además, acepto que CDF puede causar que toda o parte de estos medios se utilicen para cualquier y toda publicación, exhibición, exposición pública, editoriales y publicidad u otro propósito.
Yo renuncio cualquier derecho de inspeccionar o aprobar los medios, anuncios o publicidad en la cual el nombre de mi hijo/a, voz, apariencia, semejanza, narrativo, o comentario pueda aparecer. Yo expresamente libero y acuerdo mantener libre de culpa a CDF y sus agentes, empleados, licenciatarios y asignados de y contra cualquier y todo reclamo incluyendo, pero no limitado, a lo que yo pueda constituir como invasión de privacidad de mi hijo/a en relación a su entrevista o su uso.
